“Why have I written this book? On many occasions over the years I have been asked whether or not I have written up all the closely observed cases that have informed my career. The memories of the people who these cases represent live with me constantly whether their treatment was a success or a failure. I would like the legacy of this experience to help people in the future when I am no longer able to directly share the information myself.In particular it is important to document the successful techniques that have disappeared due to adherence to overarching dogma.
