“Why have I written this book? On many occasions over the years I have been asked whether or not I have written up all the closely observed cases that have informed my career. The memories of the people who these cases represent live with me constantly whether their treatment was a success or a failure. I would like the legacy of this experience to help people in the future when I am no longer able to directly share the information myself.In particular it is important to document the successful techniques that have disappeared due to adherence to overarching dogma. Not every medical or surgical approach is suitable for “double blind randomised crossover trials” but this does not mean that the treatment was wrong or that it did not work. In addition the repurposing of inexpensive drugs for different purposes has been a continuing theme throughout my career. Please read the book and judge for yourself.”
